Physicians who trained in the Psychiatry Postgraduate Program at the University of Toronto may request verification of training taken in the Department of Psychiatry.
The cost for Verification of Training is $100. Please send payment by money order or certified cheque in Canadian or U.S. Funds, payable to the “Department of Psychiatry, University of Toronto” and mail or courier to the following address:

Payment is required upon receipt of the Verification of Training form. We will not process the verification until payment is received. Please ensure that the payment is sent with the signed consent to release information and allow 10 business days after receipt of payment and consent for the verification.
